Request for Change of Dealer
Whenever an end user needs to change their Channel Partner of record, OpenEye requires notification and authorization to adjust our records and assist with moving the account (where applicable).
Once initiated, a request form will be sent out via DocuSign for signature. Once we receive the completed form, a waiting period of 10 days begins to allow all parties to be notified and any outstanding business items to be addressed. After the waiting period ends, OpenEye will adjust the dealer of record and move the account (where applicable).
The following information should be collected and provided on the forms, once sent to an authorized representative:
- An OWS inventory report or list of serial numbers being moved
- An authorized OpenEye reseller for the account to be transferred to

NOTE: An authorized account user is required to sign the DocuSign forms.
NOTE: OpenEye is only able to move entire accounts and cannot separate individual devices from their existing account. If moving individual devices, they should be unregistered from their current account and registered manually to the new account.
